Report: Preventable failures were 'immediate causes' of Deepwater disaster

Several "specific and preventable human and engineering failures were the immediate causes" of the BP Deepwater Horizon disaster last year, the presidentially appointed Oil Spill Commission said in its much-awaited report Tuesday .

The event "was almost the inevitable result of years of industry and government complacency and lack of attention to safety. This was indisputably the case with BP, Transocean, and Halliburton, as well as the government agency charged with regulating offshore drilling - the former Minerals Management Service," said commission co-chairman William K. Reilly.

"As drilling pushes into ever deeper and riskier waters where more of America's oil lies, only systemic reforms of both government and industry will prevent a similar, future disaster."

The report, "Deep Water: The Gulf Oil Disaster and the Future of Offshore Drilling," proposed "comprehensive" government and industry actions "to overhaul the U.S. approach to drilling safety and greatly reduce the chances of a similar, large scale disaster in the future."

    I hope that we, the taxpayers, didn't have to pay for this poll. This is a no brainer. I even talked to a deep water diver who says that they have never inspected thousands of welding repairs that, technically, are all supposed to be inspected after the fact. It has been pretty clear that ALL of these companies were in position from day one to blame any tragedy or mishap on the other guy should anything occur. Like three bad brothers. So, tell us something that we didn't know.

    1) It isnt the fault of industry. Their mission is to raise revenue and cut costs.
    2) It is the Feds fault for not making sure the industry was acting properly.
    3) Regulation is bad.

    They were finally able to plug the leak by injecting a weeks worth of John McCain's soiled grampy diapers into the ruptured well head. But I don't understand why they didn't think of it weeks sooner.

    Umm... Duh?!? When something like this happens it almost always falls one of to ways: Either complacency and unsafe practices, or intentional neglect and cost cutting by manufacturers of the products used. This time it was the unsafe practices. Next time (and as long as we are drilling there will be a next time) it will be faulty parts from the manufacturer.

    This smells like the oilfield version of the '9/11 Commission Report' where the guilty parties do not have to pay for damages. What they're not telling you is that Halliburton has already been indemnified (held-harmless with no obligation to pay for damages) even though ALL the evidence points at a faulty casing cement job. Plumes of oil were videographed spewing from the ocean floor SIX miles away from the wellbore. By who? None other than one of Jaque Cousteu's kid's. It was even shown on TV news broadcats. ONLY one thing could possibly account for this: Halliburton's faulty casing cement job(s). No wonder the day after this video evidence was shown, it became illegal for any private research vessels to get any closer than 60 miles away from the wellbore. (reminds me of Marvin Bush's Securacom corporation being dissolved the day after 9/11. They had Trade Tower security contracts, and were never investigated) These sorts of atrocities are NOT preventable as long as Big Oil, Big Pharma, ...(cont)

The World's major governments have combined with the World's major corporations to form one big Worldwide corporate fascist regime. If your country is small, you will starve and/ or be invaded if you have natural recources. Borders fall for profit but NOT for people!
